Many older adults (and vulnerable persons) face challenges related to loneliness, caregiver stress, limited mobility and difficulty accessing available services. Life Care Network was established to help address these challenges by strengthening community connections and improving access to practical, compassionate support
We build partnerships with health care providers, municipalities, faith communities, senior-serving organizations and other community agencies to strengthen referral networks, improve coordination of services and expand the range of supports available to seniors
